Long-Term Care

Long-term care services include personal in-home care, memory care, companion care, and live-in care. Each plan of care is designed to fit the individual’s needs, with regular updates to the services and schedule as needs change. Individuals who receive long-term care will receive a variety of services:

Who is it for?

Long-term care at home serves individuals who are aging in place, have a disability, have cognitive impairment, or are impacted by chronic illness. Long-term home care supports people who want to remain in their own homes for as long as possible by assisting with activities of daily living.
